Percutanous Tube Thoracostomy Procedure Note
Indication: Pleural Effusion

A time-out was completed verifying correct patient, procedure, site. The patient was positioned appropriately for chest tube placement. The patient’s <right/left> chest was prepped and draped in sterile fashion. 1% Lidocaine was used to anesthetize the surrounding skin area. A needle was inserted into the pleural space after identifying an adequate area via US. A guidewire was introduced into the pleural space. A <2 cm> skin incision was made in the mid-axillary line. The tract was dilated and a catheter was inserted over guidewire. There was rush of fluid on placement. The chest tube was sutured securely to the skin and a sterile dressing applied.
The patient tolerated the procedure well and there were no complications. Post Procedure CXR confirmed placement with no pneumothorax
