Time Out: Appropriate "time-out" procedure completed. See Time-Out flowsheet documentation.

Sedation & Analgesia:

A running IV line was present and adequate sedation was achieved with Propofol. Analgesia was given with PRN Fentanyl. ***

Technique:

Patient was positioned and prepped in usual fashion. The ventilator was set to full assist mode with 100% FIO2 for the procedure. The bronchoscope was advanced thru existing ETT. 1% topical lidocaine delivered at main carina, left main bronchus and right main bronchus. Initial inspection done in all main airways to the subsegmental branches. BAL obtained from the ***.

Final inspection and clean out done. Tip of ETT adjusted to 2-3cm above the main carina at the end of the procedure. Patient tolerated the procedure well. Vital signs remained stable. Oxygen saturations maintained above 92% throughout procedure.
