Indwelling Pleural (Aspira) Catheter Procedure Note

PREOPERATIVE DIAGNOSIS: Recurrent *** pleural effusion.
POSTOPERATIVE DIAGNOSIS: Recurrent *** pleural effusion.
PROCEDURE PERFORMED: Right indwelling pleural catheter catheter placement.
SURGEON: Christian Castaneda
ANESTHESIA: Local with IV sedation.
COMPLICATIONS: None.

DESCRIPTION OF PROCEDURE:
The patient was brought to the endoscopy suite and sat upright. The right/left*** chest was visualized by ultrasound pre-operatively and the site was marked. No IV sedation was given. The left*** chest was prepped and draped in the usual sterile fashion. Lidocaine 1% was used to infiltrate two areas; one in the anterior axillary line where the tube would exit and the other along the mid-posterior axillary line in the seventh intercostal space.
A small counterincision was made anteriorly. Through the posterior axillary line area, the pleural space was accessed by Seldinger technique. The incision was then made around the guidewire and the the Aspira catheter was tunneled retrograde from anterior to posterior. The tract was serially dilated, then a breakaway sheath introducer was passed over the wire and then the Aspira catheter was placed through the sheath introducer. There was good return of fluid.
One liter of serous fluid was withdrawn slowly as the small counterincision was closed with Monocryl stitch. There were no bleeding complications, and the fluid was not blood tinged.
The catheter was capped off, and sterile dressings were applied. The patient tolerated the procedure well without any complications. The patient was transferred to the recovery room in stable condition.
Post procedure CXR revealed ***
Patient to be sent home with 5 Aspira Catheter bags. Explained drainage procedure and dressing procedure.
Instructed to keep site clean and dry to prevent infection.
Insurance authorization form faxed to Aspira/DME company for continued bag replacements.
